Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care in the patient's home.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, communicates patient needs and changes in condition to the team.
• Function in diverse home settings with respect to age, race, creed, color, sex, disability, and financial condition.
