Job summary

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state law and care plan.
• Documents patient visits thoroughly in the EMR and verifies visits with the agency system.
• Participates in care coordination, communicates with patient, family, physician and team.
• Functions with respect to diverse patients and settings in the home environment.
