Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record per regulatory requirements.
• Participates in care coordination and communicates patient needs with the care team.
• Ability to work in diverse home situations and with patients from different backgrounds.
