Elite Home Health, part of the LHC Group and Optum family, is hiring a Licensed Practical Nurse to provide in-home patient care in the United States. You will deliver skilled nursing services, coordinate with the care team, and support families in managing care plans.

This role emphasizes compassionate bedside care, adherence to regulatory standards, and thorough documentation. A valid LPN license and reliable transportation are required; CPR is preferred.

Formación

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Preferred: home care experience and ability to work independently.

Responsabilidades

• Provides high quality clinical services within the scope of practice, as defined by the state laws, in accordance with the plan of care.
• Documents patient visits thoroughly in the electronic medical record per regulatory requirements.
• Participates in the agency's care coordination process including reporting needs and changes in condition.
• Ability to work in home settings regardless of client background.
