Job summary

Idaho Home Health, part of the LHC Group and Optum family, is hiring a Licensed Practical Nurse to provide in-home patient care. The role emphasizes high-quality clinical services, adherence to care plans, and coordination with the health team.

You will document visits in the EMR, participate in care coordination, and adapt to diverse home environments. Required: unrestricted LPN licensure and reliable transportation; CPR and home-care experience are preferred.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification (preferred) or ability to obtain within 90 days.
• Home care experience (preferred).
• Ability to manage multiple tasks and work independently.
• Good communication, writing, and organizational skills.
• Flexible hours as required to meet client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the EMR according to regulatory and payer requirements.
• Participates in care coordination, reporting patient needs and changes in condition to the team.
• Adapt to home environments regardless of client age, race, creed, or condition.
