Job summary

Monogram Health is hiring a Care Manager – RN to support patients with chronic kidney disease in their homes. You will collaborate with nephrologists and a care team to implement care plans, monitor health data, and coordinate medications to improve outcomes.

The role emphasizes direct patient interaction, education on dialysis and transplant options, and fostering strong patient relationships and adherence to care plans. Occasional travel to support transitions is possible.

Qualifications

• Must be a graduate of an accredited School of Nursing.
• Currently licensed as a Registered Nurse in AZ or an NLC state.
• 1+ year of care management or CKD/ESRD experience preferred.

Responsibilities

• Develop and adapt individualized care plans with nephrologists.
• Perform in-home care management visits.
• Monitor biometric data and follow protocols for interventions.
• Coordinate medications with pharmacists and prescribers.
• Educate patients on CKD, ESRD, dialysis and related factors.
• Engage families and support groups in care.
