Job summary

Fairview Health Services in Minnesota is seeking a Home Health Infusion Nurse to provide in-home IV infusion therapy, nursing care, and patient education on treatment regimens and self-management when appropriate.

This casual position requires the ability to work independently, stay detail oriented, and manage multiple tasks in a fast-paced home care environment while coordinating with the care team and documenting care activities in the chart.

Qualifications

• Graduate of an accredited school of nursing.
• Proficiency in ultrasound IV placement.
• 2 years of recent nursing experience in relevant settings.
• MN RN license and BLS certification.
• Driver's license and automobile insurance.

Responsibilities

• Deliver in-home infusion care and assessments per physician guidance.
• Coordinate with patient, family and care team on care plans.
• Educate patients/caregivers on self-administration and safety.
• Coordinate discharge planning with physician and team.
• Provide care in home, infusion suites, or hospital as needed.
• Train patients on self-administration and symptom recognition.
• Maintain communication with Charge Nurse and Nursing Leadership.
• Address drug management changes with Pharmacy/Nursing Leaders.
• Document all care activities promptly in the clinical record.
