Job summary

Fairview Health Services is seeking an in-home Infusion Nurse to provide IV infusion therapy, nursing care, and patient education in home settings. The role requires independent work, attention to detail, and the ability to manage multiple tasks in a fast-paced environment.

You will deliver care delegated by physicians, coordinate with the care team, educate patients on self-administration, and participate in discharge planning across homes, infusion suites, and hospital settings.

Qualifications

• Graduate of an accredited school of nursing.
• Must hold MN RN license.
• Basic Life Support certification required.
• Proficient with computer applications (EMR, Outlook).
• Valid driver's license and automobile insurance.

Responsibilities

• Deliver nursing care and infusion therapy as ordered by physician in patient homes and other settings.
• Assess health status, document in clinical records, and communicate with care team.
• Educate patients and caregivers on self-administration, storage, and safety.
• Coordinate discharge planning with physician and care team.
• Provide care in homes, infusion suites, and hospitals as needed.
• Support timely communication with Nursing Leadership and scheduling.
