Heart of Hospice is seeking a Social Worker to provide in‑home medical social services under the direction of a physician and the interdisciplinary team. The role involves understanding social and emotional factors related to health and helping patients develop coping strategies while collaborating on care plans.

This full‑time position requires an MSW from a CSWE-accredited program and a current Social Worker license in the state.

Qualifications

• MSW from a CSWE-accredited program.
• Current CPR certification or ability to obtain within 90 days.
• Licensed Social Worker in the state of residence.

Responsabilités

• Assist patients, families, physician, and care team in understanding social factors affecting health status.
• Contribute to a comprehensive, integrated Plan of Care on a daily basis.
• Instruct care team on community resources available to assist patients.
