Job summary

Heart 'n Home Hospice, part of LHC Group, seeks a Licensed Social Worker to support patients on an interdisciplinary team under physician supervision. The role focuses on understanding social and emotional factors impacting health and developing coping strategies.

The position involves collaborating with patients, families, and care teams to craft a Plan of Care and connecting patients with community resources. A Master’s in Social Work and OR licensure are required.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• OR: Licensed as a Social Worker from the State Board of Social Work.

Responsibilities

• Help patients and families understand emotional and social factors affecting health.
• Contribute to a comprehensive, integrated Plan of Care for patients.
• Instruct the team on community resources available to assist patients.
