Job summary

UnitedHealth Group’s Heart n Home Hospice in Oregon is seeking a Licensed Social Worker to join our care team. You will provide medical social services under physician and Interdisciplinary Group guidance to support patients, families, and caregivers.

Responsibilities include assessing social and emotional factors, contributing to the Plan of Care, and guiding staff on community resources. Minimum MSW and Oregon licensure; CPR certification preferred.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council on Social Work Education.
• Current Driver's License and access to reliable transportation.

Responsibilities

• Provide medical social services under the direction of a physician and Interdisciplinary Group.
• Assess patient and family social, emotional, and environmental factors related to health status.
• Contribute to a comprehensive Plan of Care and guide staff on community resources.
