Job summary

Heartlite Hospice is seeking a Social Worker to provide medical social services under physician direction and the Interdisciplinary Group, addressing social and emotional factors affecting patient health and coping strategies.

You will help develop an integrated Plan of Care, educate the team on community resources, and support patients and families in navigating care options. AL licensure and CPR readiness bolster candidacy.

Qualifications

• Master’s Degree in Social Work required.
• Licensed Social Worker in AL preferred.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instructs health care team members on community resources available to assist patients on a as needed basis
