Enhabit Home Health & Hospice is seeking a licensed social worker to provide social services to patients in home health and hospice settings. You will educate patients and families, coordinate with the care team, and contribute to the planned care under physician orders.

Candidates should hold an MSW, be licensed in the state, and have at least one year of health care social work experience. Medicare/home health experience is a plus.

Qualifikationen

• Must have a master’s degree from a school of social work accredited by the council on social work education.
• Must be certified or licensed in the state where they currently practice.
• Must have at least one year of social work experience in a health care setting.
