Job summary

Community Hospice Inc is seeking a Registered Nurse for Homecare to provide skilled nursing care and ongoing assessment for patients in home settings. You will plan, implement, and evaluate nursing care, and participate in on-call coverage including evenings, weekends, and holidays as per policy.

Qualifications

• Must be a graduate of an approved school of nursing with current licensure in Kentucky and Ohio.
• Continuing education requirements will be maintained according to Kentucky and Ohio licensure requirements.
• Two years’ experience in a patient care position in a hospital, nursing home, or related health care facility.

Responsibilities

• Functions within standards of Nurse Practice Act.
• Completes initial assessment of home care needs, including physical, emotional and mental status.
• Visits assigned patients weekly for physical, emotional and pain assessment.
• Prepares and implements treatment plan for patient and family at IDT meetings; updates as needs change.
• Provides direct nursing care to patients to maintain comfort and quality of life.
• Maintains contact with primary physician and documents physician orders accurately.
• Assists in coordinating services with other agencies involved in patient care.
• Provides a safe patient care environment including universal precautions.
• Orders medications, supplies and equipment as needed.
• Teaches caregivers proper use of medications and equipment.
• Maintains accurate records and communicates changes to team and supervisor.
• Supervises LPN/CNA assisting with patient care and reports difficulties to Director of Clinical Services.
• Responsible for patient recertification and ensuring continued eligibility for hospice services.
