Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to provide compassionate, patient-centered care in home and community settings. You will deliver skilled nursing, develop plans of care, and support families through the hospice journey.

Responsibilities include assessments, coordination with the IDG, and thorough documentation. An RN license, 1 year hospice/home health experience, CPR, and a valid driver's license are required.

Qualifications

• RN license in the state of employment
• Minimum 1 year RN experience in hospice, home health, or related setting
• CPR certification required
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and education for patients, families, and caregivers
• Deliver prudent nursing interventions and symptom management aligned with goals of care
• Develop and update individualized plans of care including patient visit frequency
• Coordinate care across interdisciplinary teams with physicians and nurses
• Provide patient and caregiver education on disease process, self-care, end-of-life care, medications and nutrition
