UnitedHealth Group is seeking an RN Case Manager to coordinate compassionate hospice care in Chesapeake, VA. You will assess patients, work with the interdisciplinary team, and develop individualized care plans to meet each patient’s needs.

The role emphasizes teaching families, documenting progress, and communicating with the care team, with on-call duties as required. A valid RN license and CPR are required.

Qualifications

• 1+ years nursing experience required.
• Current RN license for the applicable state(s).
• CPR certification required.
• Valid driver's license with reliable transportation and liability insurance.

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records accurately and timely.
• Records pain and symptom management changes/outcomes as appropriate.
• Teaches patients, families and caregivers appropriate care techniques.
• Regularly communicates with clinical manager and care team.
• On-call responsibilities and services to patients/families as assigned.
• Other duties as assigned.
