Job summary

Ennoblecare is seeking a full-time Hospice Registered Nurse Case Manager to work in Union / Essex County, NJ. The RN will provide skilled nursing assessments and care to maximize comfort for patients and their caregivers. You will be responsible for managing a patient caseload, coordinating with the attending physician, and documenting care in electronic medical records.

The ideal candidate will be a graduate from an accredited nursing school, licensed in New Jersey, with at least one year of RN experience. Benefits include medical, dental, and paid time off.

Qualifications

• Must have one year of RN experience; hospice experience preferred.
• Licensed driver with an insured automobile in good working order.

Responsibilities

• Work under the general direction of the Director of Nursing.
• Assume primary responsibility for patient caseload management.
• Provide accurate and timely documentation in electronic medical records.
• Participate in interdisciplinary group meetings.
