Job summary

Ennoble Care in Cumberland County, NJ is recruiting a full-time Registered Nurse Case Manager to service hospice patients at home. You will assess, plan, and coordinate care with the interdisciplinary team under the Director of Nursing and attending physician.

You’ll communicate with physicians, Hospice staff, and external agencies to maximize comfort and health for patients and families. The ideal candidate is a graduate of an accredited school of nursing, licensed in NJ, with at least one year

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Must be licensed in NJ as a Registered Nurse.
• One year of RN experience; hospice experience preferred.
• Excellent assessment skills and problem solving abilities.
• Strong organization and time-management capabilities.
• Must have a valid driver's license with a reliable vehicle.

Responsibilities

• Manage a patient caseload with assessment, planning, implementation and evaluation.
• Coordinate with attending physicians, Hospice staff and external agencies for optimal patient care.
• Maintain timely documentation in electronic medical records.
• Participate in Interdisciplinary Group meetings and care conferences.
• Support caregivers and coordinate nursing care during on-call periods.
