Job summary

Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to serve Somerset County, NJ. You will perform skilled nursing assessments, plan and coordinate care with an interdisciplinary team, and ensure comfort and health for patients and their families.

Responsibilities include caseload management, communication with physicians and hospice staff, accurate EMR documentation, and participation in team meetings.

Qualifications

• Current NJ RN license required.
• 2+ years as a professional nurse, hospice preferred.
• Ability to assess needs and communicate with patients and families.

Responsibilities

• Manage a patient caseload with nursing assessment, planning, implementation and evaluation.
• Coordinate care with attending physician and hospice team.
• Maintain timely documentation in EMR and participate in IDG meetings.
• Provide support to patients, families, and caregivers, including bereavement support.
• Collaborate with interdisciplinary team and arrange for appropriate resources.
