Job summary

Ennoble Care is hiring a full-time Hospice Registered Nurse Case Manager to serve the Camden County, NJ region. The RN Case Manager collaborates with the interdisciplinary team to assess, plan, and implement patient-centered care aimed at comfort and quality of life.

The ideal candidate will be a graduate of an accredited nursing program, hold a NJ RN license, have at least one year of RN experience (hospice preferred), and be bilingual in English and Spanish.

Qualifications

• Must be a graduate from an accredited School of Nursing.
• One year of RN experience. Hospice experience preferred.
• Must have excellent assessment and problem-solving skills.
• Must be a licensed driver with an insured automobile.
• Bilingual in English and Spanish.

Responsibilities

• Manage patient caseload with assessment, planning, implementation and evaluation.
• Coordinate with attending physicians, Hospice staff and other agencies to optimize patient care.
• Document accurately in the EMR and participate in Interdisciplinary Group meetings.
• Provide support for primary caregivers and bereaved families as part of care.
