Ennoble Care is seeking a Full-time Hospice Registered Nurse Case Manager to cover Cincinnati, OH and surrounding areas. The role involves assessments, planning, and coordinating home-based hospice care to maximize patient comfort and support families.

The RN will work under the Director of Nursing and collaborate with an interdisciplinary team, ensuring timely documentation and adherence to policies. A valid driver’s license and year-round access to reliable transportation are required.

Qualifications

• Current RN license (state of GA per posting).
• 1–2 years nursing experience; hospice preferred.
• Ability to assess and respond to patient and caregiver needs in varied settings.
• Skilled in nursing practice with empathy for families.

Responsabilités

• Perform skilled nursing assessments and care planning.
• Coordinate with attending physicians and Hospice team for optimal patient care.
• Communicate with patients, families, and caregivers regularly.
• Document accurately in electronic medical records.
• Participate in interdisciplinary group meetings and care conferences.
• Provide bereavement and end-of-life support as appropriate.
