Job summary

Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to serve the Dayton, OH area. You will work with an interdisciplinary team to provide skilled nursing assessments, planning, and care to maximize comfort for patients and families.

The role requires an active RN license (GA) and 1–2 years of hospice experience, with strong communication and documentation skills. Eligible for standard benefits for full-time employees and a driver's license for home visits.

Qualifications

• Current license to practice as a registered nurse in GA.
• 1–2 years of professional nursing experience, preferably hospice.
• Ability to assess patient and caregiver needs in varied settings.

Responsibilities

• Identify patient and caregiver needs and provide supportive care per physician orders and plan of care.
• Manage a patient caseload including assessment, planning, implementation, and evaluation of nursing care.
• Communicate with attending physicians, hospice staff, and other agencies to coordinate optimal care.
• Maintain regular documentation in electronic medical records.
• Participate in Interdisciplinary Group meetings and care conferences.
• Supervise and document NAC every two weeks or as assigned.
• Occasionally fill in for primary nurses during off-hours to meet patient and family needs.
