Job summary

Ennoble Care is seeking a Full-time Hospice Registered Nurse Case Manager to serve Gwinnett, GA and surrounding areas. The RN Case Manager will work with the interdisciplinary team to provide skilled nursing assessments, planning, and care under supervision of the Director of Nursing and attending physician.

Responsibilities include caseload management, coordination with hospice staff and other agencies, timely EMR documentation, and participation in care conferences.

Qualifications

• Must hold a current GA RN license.
• 1–2 years of professional nursing experience, preferably in hospice.
• Ability to assess patients and respond to caregiver needs.
• Skilled in nursing documentation and reporting.
• Team player with the interdisciplinary hospice approach.

Responsibilities

• Manage a patient caseload and oversee nursing process steps (assessment, planning, implementation, evaluation).
• Coordinate care with attending physicians, Hospice staff, and other agencies.
• Maintain timely documentation in electronic medical records.
• Contribute to Interdisciplinary Group meetings and care planning.
• Provide support at time of death and bereavement as needed.
