Job summary

St. Francis Healthcare System of Hawai‘i seeks an RN for Hospice Home Care to deliver direct and indirect nursing care in patients' homes, following the Hospice Standards for Nursing Practice.

You will work with an interdisciplinary team to create and implement individualized plans, addressing the needs of patients and families in familiar surroundings. The role requires Hawaii RN licensure, with 2+ years hospice experience preferred.

Qualifications

• Registered Nurse; BSN preferred but not required.
• Current Hawaii RN license required.
• Minimum two years in hospice care preferred.
• Reliable transportation with valid license, clean driving record, and car insurance.

Responsibilities

• Provide total nursing care to hospice patients in the home setting.
• Coordinate care with the interdisciplinary team including physicians and social workers.
• Administer and supervise the patient care plan per Hospice standards.
• Arrange for equipment and medication delivery as needed.
• Instruct families in basic nursing procedures and home care tasks.
• Maintain a safe and supportive environment for patients and families.
