St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1 holistic home-based care to patients and families, coordinating with an interdisciplinary team to create individualized care plans.

You will guide end-of-life conversations with compassion, educate patients and caregivers, and participate in on-call rotations. RN required, with acute care experience; hospice/home health preferred; drivers license and reliable transportation required.

Qualificações

• Associates, Bachelors, or Masters in nursing (RN).
• 1+ year of acute care experience; hospice/home health preferred.
• Complete initial and ongoing comprehensive hospice assessments.
• Develop individualized care plans with patient and family.
• Must have reliable transportation and valid driver's license.

Responsabilidades

• Provide 1:1 holistic home-based care to patients and families.
• Develop individualized care plans with the patient and family, updating as needs evolve.
• Guide end-of-life conversations with professionalism and empathy.
• Rotate on-call duties and ensure seamless care transitions.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
