St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to provide 1:1, whole-person care to patients and families in their homes.

You will collaborate with an integrated care team to develop and update individualized care plans, educate families, and support them through difficult decisions. The role requires an RN with an accredited nursing degree and at least 1 year of acute care experience; hospice/home health experience is preferred.

Qualifications

• RN license required; board-eligible in state.
• 1+ year of acute care experience; hospice/home health preferred.
• Associates, Bachelors, or Masters in nursing from an accredited program.

Responsabilités

• Provide 1:1, whole-person care in patients' homes.
• Develop individualized care plans with patients and families.
• Coordinate with an integrated care team; support families through decisions.
• Administer medications and treatments as prescribed; educate patients and caregivers.
• Rotate on-call duties and ensure seamless care transitions.
