Affinity Health Management seeks an experienced registered nurse to oversee hospice care in home settings. Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing.
• RN with 1 year of experience within the last 3 years, or a Baccalaureate degree in nursing with 2 years’ experience (at least one in public health, home care or hospice preferred).
• Management experience not required; responsible for supervising hospice aides.
• Offer contingent upon satisfactory references and criminal background checks.
• Excellent observation, verbal and written communication skills, problem solving and basic math; nursing skills per competency checklist.
• Prolonged walking/standing; able to lift, position and transfer patients; able to lift supplies.
• Must be a licensed driver with insured vehicle in good working order.
• Possesses CPR certification (hands-on demonstration required).

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Writes and initiates plan of care; regularly re-evaluates patient and family/caregiver needs.
• Initiates plan of care and makes revisions as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and includes patient and family.
• Administers medications and treatments as prescribed by physician.
• Counsels patient and family; provides health care instructions as appropriate.
• Acts as Case Manager for assigned caseload; coordinates patient care.
• Completes accurate clinical notes and communicates with physicians and care team.
• Participates in interdisciplinary team meetings and quality improvement activities.
