Geisinger Hospice-Danville, part of the Optum family, seeks a Home and Community Based Services Aide to support a client in their own home with personal care, daily living activities, and medication reminders. The role involves assisting with household tasks, accompanying clients to appointments, and maintaining a safe home environment.

You will monitor client condition, document vitals using EVV, and ensure confidentiality. A valid driver’s license and PA training compliance are required.

Competenze

• Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
• PA: Competency or training program meeting 28 Pa. Code § 611.55 OR a home health aide training program meeting 42 CFR 484.36 OR a training program meeting the agency's training standards in a Medicaid Waiver or other publicly funded PCS program.

Mansioni

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
