Job summary

ARIN HOME HEALTHCARE SERVICES LLC in Columbus, OH is seeking a compassionate Registered Nurse to provide in-home patient care. You will assess patients, develop care plans, administer medications, and collaborate with physicians and the care team to ensure quality outcomes.

The ideal candidate holds an active Ohio RN license, has at least 1 year of clinical nursing experience (home health preferred), with strong knowledge of Medicare/Medicaid home health regulations and OASIS documentation.

Qualifications

Active RN license in Ohio
At least 1 year of clinical nursing experience; home health preferred
Strong knowledge of Medicare/Medicaid home health regulations and OASIS documentation
Valid driver’s license and reliable transportation for home visits in Columbus, OH
Excellent clinical assessment, critical thinking and problem-solving skills
Strong communication and ability to work independently within a multidisciplinary team
CPR/BLS certification required

Responsibilities

Conduct comprehensive patient assessments and develop individualized home health care plans
Administer medications, wound care, IV therapy and other skilled nursing treatments at home
Monitor patient health, document changes and communicate with the care team
Educate patients and families on disease management, medications and self-care techniques
Coordinate with physicians, therapists and others to ensure continuity of care
Maintain accurate clinical documentation per Medicare/Medicaid and agency standards
Participate in care conferences and contribute to discharge planning and goals
