Job summary

• Valid driver’s license and reliable transportation for home visits in Columbus, OH
• Excellent clinical assessment, critical thinking and problem-solving skills
• Strong communication and ability to work independently within a multidisciplinary team
• CPR/BLS certification required

Responsibilities

• Conduct comprehensive patient assessments and develop individualized home health care plans
• Administer medications, wound care, IV therapy and other skilled nursing treatments at home
• Monitor patient health, document changes and communicate with the care team
• Educate patients and families on disease management, medications and self-care techniques
• Coordinate with physicians, therapists and others to ensure continuity of care
• Maintain accurate clinical documentation per Medicare/Medicaid and agency standards
• Participate in care conferences and contribute to discharge planning and goals

Requirements:

• Minimum 1 year of clinical nursing experience; home health experience preferred
• Strong knowledge of Medicare/Medicaid home health regulations and OASIS documentation
• Valid driver's license and reliable transportation for home visits throughout Columbus, OH
• Excellent clinical assessment, critical thinking, and problem-solving skills
• Strong communication skills with the ability to work independently and as part of a multidisciplinary team
