Job summary

Visiting Nurse Association of Florida is hiring a Registered Nurse (RN) for Home Health in the Boynton Beach, Delray Beach, Boca Raton, and S West Palm Beach areas of FL. This full-time position offers meaningful home-based care with a trusted nonprofit leader serving Florida for 50 years.

You will perform skilled nursing visits, complete OASIS assessments, and develop patient-centered care plans while educating families on disease management and safety.

Qualifications

• Active Florida RN license
• Minimum one year of nursing experience
• Home health experience preferred
• OASIS experience preferred
• Strong assessment and critical thinking skills
• Ability to work independently

Responsibilities

• Perform skilled nursing visits in patient homes
• Complete OASIS assessments and documentation
• Develop and manage individualized care plans
• Educate patients and caregivers on disease management and safety
• Coordinate care with physicians and interdisciplinary team members
• Monitor patient progress and communicate changes in condition
