Job summary

Visiting Nurse Association of Florida (VNA) seeks a Registered Nurse for home health in Palm Beach County. This PRN role offers flexible scheduling to serve patients in their homes, delivering skilled nursing visits and care coordination in a trusted nonprofit hospital-at-home model.

The RN will perform assessments, develop care plans, educate patients and families, and collaborate with the interdisciplinary team to ensure quality outcomes.

Qualifications

• Active Florida RN license.
• Minimum one year of nursing experience.
• Home health experience preferred.
• OASIS experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently.

Responsibilities

• Perform skilled nursing visits in patient homes.
• Complete OASIS assessments and documentation.
• Develop and manage individualized care plans.
• Educate patients and caregivers on disease management and safety.
• Coordinate care with physicians and interdisciplinary team members.
• Monitor patient progress and communicate changes in condition.
