Job summary

VNA of Florida is seeking a Registered Nurse to provide skilled home health visits across Boynton Beach, Delray Beach, Boca Raton, and surrounding areas. You will perform visits, complete OASIS assessments, and coordinate with a multidisciplinary team to support patients at home.

The ideal candidate holds an active Florida RN license, has at least one year of nursing experience (home health preferred), and demonstrates strong assessment skills and independent work ethic.

Qualifications

• Active Florida RN license.
• Minimum one year of nursing experience.
• OASIS experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently.

Responsibilities

• Perform skilled nursing visits in patient homes.
• Complete OASIS assessments and documentation.
• Develop and manage individualized care plans.
• Educate patients and caregivers on disease management and safety.
• Coordinate care with physicians and interdisciplinary team members.
• Monitor patient progress and communicate changes in condition.
