Job summary

VNA of Florida is seeking a Registered Nurse (RN) to join our Home Health team in Palm Beach County on a PRN basis. The RN will perform skilled nursing visits in patients’ homes, complete OASIS assessments, and develop individualized care plans.

The role also involves educating patients and caregivers on disease management and safety, coordinating care with physicians, and monitoring patient progress with the interdisciplinary team.

Qualifications

• Active Florida RN license.
• Minimum one year of nursing experience.
• Home health experience preferred.
• OASIS experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently.

Responsibilities

• Perform skilled nursing visits in patient homes
• Complete OASIS assessments and documentation
• Develop and manage individualized care plans
• Educate patients and caregivers on disease management and safety
• Coordinate care with physicians and interdisciplinary team members
