Job summary

Residential Home Health and Hospice is hiring a Physical Therapist Assistant (PTA) for home health in Aurora, Oswego, Montgomery and surrounding areas. Eligible for a $5,000 first year success bonus based on key achievements within the first year.

Provide compassionate, patient-centered PT services under a licensed PT, helping patients restore mobility and independence through exercises and gait training. Travel to patient homes within assigned territory is required.

Qualifications

• Active Physical Therapist Assistant license in the state of work.
• Minimum one year of PTA experience delivering patient care.
• Current driver's license and ability to drive to patient homes ~20% of day.

Responsibilities

• Provide compassionate, patient-centered Physical Therapy services in-home under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises, gait training, transfer training, muscle re-education, and prosthetic or orthotic support.
• Educate patients and caregivers on the safe use of mobility and assistive devices, while monitoring progress and communicating patient responses and condition changes to the care team.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
