Job summary

Residential Home Health and Hospice is hiring a Physical Therapist Assistant (PTA) for home health in the Springfield area. The role focuses on providing skilled PTA services under a licensed PT, helping patients improve mobility and independence.

Expect to educate patients and families, document thoroughly in EMR, and travel to patient homes within the assigned territory. A first-year $5,000 success bonus is offered, with ongoing opportunities as you grow with the company.

Qualifications

• Active Physical Therapist Assistant (PTA) license in the state you work.
• At least one year of PTA experience delivering patient care.
• Current driver’s license and ability to drive ~20% of day.

Responsibilities

• Provide PTA services in home health under the direction of a licensed Physical Therapist.
• Support patients in mobility restoration and functional independence through exercises and gait training.
• Educate patients and caregivers on safe use of mobility devices and monitor progress.
• Maintain thorough, compliant EMR documentation.
• Travel to patient homes within assigned territory.

What You’ll Do:

• Provide compassionate, patient-centered Physical Therapy services in the home health setting under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises, gait training, transfer training, muscle re‑education, and prosthetic or orthotic support.
• Educate patients and caregivers on the safe use of mobility and assistive devices, while monitoring progress and communicating patient responses and condition changes to the care team.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PTA Qualifications

• Active Physical Therapist Assistant, PTA license in the state you work
• One year of experience as a Physical Therapist Assistant and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
