Job summary

Residential Home Health and Hospice is seeking a Physical Therapist Assistant (PTA) to deliver skilled services in patients' homes in the Aurora, Oswego, Montgomery area. You will work under a licensed Physical Therapist, focusing on mobility, pain reduction, and functional independence through targeted exercises and gait training.

The role requires a PTA license in Illinois, a valid driver's license, and about 20% travel time.

Qualifications

• Active Physical Therapist Assistant license in IL.
• Minimum 1 year PTA experience.
• Valid driver’s license and ability to travel to patient homes.

Responsibilities

• Provide PTA services in home health under PT supervision.
• Assist patients to regain mobility and independence through therapeutic exercise.
• Educate patients and caregivers on safe mobility and device use; document progress in EMR.
