Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

You will join a national network of clinicians, travel locally and within licensed states, and use an iPad-based workflow to conduct comprehensive evaluations.

Qualifications

• Active, unrestricted medical license in the United States.
• MD or DO degree; board certification preferred.
• Multistate licenses preferred.

Responsibilities

• Provide in-home health evaluations to members.
• Engage with patients and families to address health needs.
• Utilize iPad-based clinical workflow; perform assessments.
• Travel locally and within licensed states as needed.
• Document findings and coordinate with health plan care teams.
