Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations and bridge gaps in care. • Multi-state licenses preferred.
• MDs and DOs eligible to apply.

Responsibilities

• Conduct in-home health evaluations during 45‑minute visits.
• Build trusted relationships to improve health outcomes.
• Travel locally and within licensed states as needed.
• Use Signify Health services including diagnostic & preventive care.
• Perform virtual visits when required.
• Capture health data, discuss plans and coordinate with health plans.
