Job summary

Signify Health, part of CVS Health, is seeking a per diem physician to perform in-home health evaluations for members in their own homes. The role involves connecting with patients, reviewing medical histories, and providing clinical assessments without prescribing or altering treatment plans.

The role supports flexible scheduling and travel within licensed states. The position is part-time and per diem, with an hourly base rate and incentive opportunities.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Visit individuals in their homes to conduct in-home health evaluations.
• Engage with patients to answer health questions and provide assessments.
• Coordinate with health plans to bridge gaps in care.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Provide diagnostic & preventive services as indicated.
