Job summary

Signify Health invites MDs or DOs to join our network as Per Diem Physicians for in-home health evaluations. You will perform comprehensive visits at patients’ homes, including history review, physical assessment, and coordination with care teams.

The role emphasizes flexible scheduling, travel within licensed states, and the option for virtual visits where appropriate. Candidates must hold an active medical license and preferably multi-state licensure.

Qualifications

• Active medical license in coverage area(s).
• Multi-state licenses preferred.
• In-home clinician role with the Signify Health network.

Responsibilities

• Visit patients in their homes with a focus on health risk assessment and care coordination.
• Travel locally within licensed states as needed; conduct in-home and virtual visits.
• Provide Signify Health diagnostics and preventive services per program guidelines.
• Engage with health plan members to connect them with the right care at the right time.
