Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations across participating states. You’ll visit patients in their homes, perform clinical assessments, review medications and histories, and connect members to the right care.

The role requires MD or DO with active licenses, multi-state licenses preferred, and willingness to travel locally. Expect 40 hours per week with comprehensive benefits and competitive pay in the $175/hour range.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multistate licenses preferred.

Responsibilities

• Provide in-home health evaluations and comprehensive assessments.
• Review medications and medical histories with patients.
• Bridge gaps in care by connecting members to appropriate services.
• Travel locally to visit patients within licensed states; conduct virtual visits as needed.
• Utilize iPad and clinical workflow; follow Signify Health protocols.
