Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations, engaging with people in their homes and bridging gaps in care. This role leverages a national network of clinicians and requires travel to patients' homes across licensed states while maintaining high-quality care standards.

You will perform 45‑minute visits, review medications and medical history, and conduct physical evaluations.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations with ~45‑minute visits.
• Build trusted relationships to improve health outcomes.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Coordinate with health plans to connect patients with the right care.
• Perform diagnostic tests and screenings as ordered by the plan.
