• Board certification required

Responsibilities

• Provide In-Home Health Evaluations in patients' homes
• Engage with patients to assess health status and needs
• Use iPad-based workflow for evaluations
• Travel locally to licensed states as needed
• Conduct virtual visits when required
• Provide Signify Health services including diagnostic and preventive assessments
