Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to perform In-Home Health Evaluations, visiting patients in their homes across licensed states. You’ll assess health status, review meds and history, and connect people with the right care.

This part-time role averages around 29.5 hours weekly with flexible scheduling and a strong focus on patient education, preventive services, and health plan alignment.

Qualifications

• Active, unrestricted licenses in coverage areas.
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Bring your heart into every visit with a national network of clinicians.
• Visit patients nationwide to connect them with the right care.
• Work with Signify Health as a clinician colleague.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide Signify Health services including Diagnostic & Preventive Services.
