Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. The role involves visiting members at home, reviewing medications and history, and performing a brief physical examination.

You will collaborate with a national clinician network and may conduct virtual visits as needed. The position offers approximately 29.5 hours per week, with a pay range of $41.85–$90.13 per hour.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Visit millions of people nationwide to connect them with the right care
• Collaborate with Signify Health clinicians as a team member
• Be flexible to travel locally and within licensed states as needed
• Conduct virtual visits as needed
• Provide Diagnostic & Preventive Services as indicated (e.g., spirometry, bone density)
