Job summary

Signify Health, part of CVS Health, is seeking a Part-Time Clinician (NP/PA) to perform in-home health evaluations. Visit members in their homes, review medical history and perform physical assessments with a focus on bridging gaps in care.

Qualified candidates hold active licenses, board certification, and may have multi-state licenses; travel locally within licensed states is required, and virtual visits are possible. 29.5 hours per week, base pay range shown.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Conduct in-home health evaluations for members.
• Review medication and medical history; perform physical evaluation.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide diagnostic and preventive services as indicated (e.g., spirometry, bone density).
