Signify Health, part of CVS Health, is hiring physician (MD/DO) contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members in a remote/AL setting. You will perform in-home visits focused on overall health and barriers to care while earning based on completed assessments.

Shifts span Monday to Sunday with no required nights/weekends, and there is potential for pay-per-test incentives where eligible.

Qualifications

• MD/DO license and ability to conduct in-home health evaluations.
• Medicaid license or willingness to enroll with support.
• Available for flexible, in-home visits across assigned markets.

Responsabilités

• Conduct in-home visits that focus on a person’s overall health and identify potential barriers to care.
• Manage pre-scheduled in-home visits with health plan members.
• Utilize pay-per-test model where applicable for eligible visits.
• Leverage market multiplier to increase reimbursement per visit.
