Job summary

A Place at Home is seeking a dedicated Home Health Aide to support clients in their own homes. You will assist with daily living activities, administer reminders for medications, and help with meals, mobility, and light housekeeping as directed by care plans.

Qualifications

• Completion of state-approved Home Health Aide certification course.
• High school diploma or equivalent.
• Must pass a background check and drug test.
• Reliable transportation and valid driver\'s license.
• Strong communication skills, both written and verbal.

Responsibilities

• Provide assistance with ADLs such as bathing, dressing, grooming, and toileting.
• Assist with meal planning and preparation while observing dietary restrictions.
• Assist with medication reminders per care plans.
• Perform light housekeeping (dusting, vacuuming, laundry) to maintain a clean home.
• Assist with mobility and transfers, ensuring client safety.
• Provide companionship and develop a caring relationship with clients.
• Document and report changes to supervising nurse.
• Follow HIPAA guidelines and protect client PHI.
• Participate in ongoing training and professional development.
