Qualifications

• Graduate from an accredited PA program or Nursing Approved Master’s/Post Master’s program.
• Board certification in relevant specialty preferred (geriatrics, palliative care).
• Experience in home health, geriatrics, or hospice care is advantageous.

Responsibilities

• Conduct in-home, video, and telephonic comprehensive clinical assessments.
• Order and interpret labs and imaging; develop treatment plans within scope and standards.
• Manage palliative care medications and symptoms; support advance care planning.
